Payment Policy

Patients are responsible for the charges that are provided. To assist patients in meeting their financial obligations, we will bill your health insurance carrier(s) directly as long as you provide a valid ID card and information regarding active insurance coverage at the time of registration. Services may be billed by Franciscan Health and/or Otolaryngology Associates for the provider technical fee, a provider professional fee, and a facility fee.

Insurance Resources

It is the patient’s responsibility to contact their insurance carrier directly to confirm coverage for services to be provided. To assist you with this, we have listed all the CPT procedure codes used in the balance assessment services below. The results of the balance testing will be reviewed and interpreted by Dr. Benjamin Copeland, and his summary report will be sent to the referring physician.
